You will need the following documents to complete the parent section

• Last year’s 1040/1040A U.S. IRS Tax Form
• Student’s Permanent Resident Card or INS Form 1-551 or U.S. Passport if the student is not a Citizen.
• Legal Guardianship document (Students who do not live with parents)
